Pradaxa (dabigatran etexilate) is an anticoagulant medication used in several scenarios to prevent or treat blood clots.

This medication works about as well as warfarin. In studies, Pradaxa produced fewer life-threatening bleeds, but did produce slightly more gastrointestinal bleeds; patients who have a history of ulcers or gastrointestinal bleeding may not want to buy Pradaxa.

Pradaxa also has an antidote, idarucizumab, which is effective at countering Pradaxa within moments of administration. Patients who buy Pradaxa may want to carry an ID bracelet or similar notifications that the medication is being taken. If undergoing any sort of surgery---including dental surgery---Pradaxa use will likely need to stop temporarily.

Pradaxa Uses

In most cases patients buy Pradaxa to:

• Prevent deep vein thrombosis (blood clot in veins) or pulmonary embolism (blood clot in lungs) following hip replacement surgery
• Reduce risk of recurring deep vein thrombosis or pulmonary embolism in patients who have previously had them
• Treat existing deep vein thrombosis or pulmonary embolism
• Reduce risk of stroke or systemic embolism in patients with non-valvular arterial fibrillation

It may be used for other purposes than the above. Doctor instructions should be followed through the duration of treatment and doctor checkups should be attended.

Pradaxa should not be used by individuals with a prosthetic heart valve.

Note that this medication should not be ended abruptly; dosage will usually be gradually reduced.

Dosage & Administration

Pradaxa comes in tablets, either 75 mg, 110 mg, or 150 mg strengths. Tablets must be swallowed whole and not be split or crushed. Food is optional; a full glass of fluid is recommended.

Missed doses should be taken as soon as remembered unless there are 6 hours or less before the next dose, in which case it can be skipped entirely. Never take a double-dose of Pradaxa to compensate for a missed dose.

Duration of treatment is variable, with the exception of using it following hip surgery. Treatment may span months or years, depending on a wide range of personal variables.

Dosage is also variable; to a large extent it depends on kidney function. Patients should have a creatinine clearance (CrCl) test performed prior to establishing dosage.

Those who have moderate or better kidney function---CrCl greater than 30 ml/min---are usually prescribed:

• Following hip replacement surgery - 110 mg for the first day, then 220 mg once daily for 28 to 35 days.
• Preventing recurring blood clots - too variable to summarize.
• Treating existing blood clots - 150 mg taken twice per day (total of 300 mg per day).
• Reduce risk of stroke in non-valvular arterial fibrillation - 150 mg twice per day (total of 300 mg per day).

Those with severe kidney damage---CrCl less than 30 ml/min---may be given lower doses of 75 mg twice per day, or told not to buy Pradaxa at all.

Precautions

The majority of patients who use Pradaxa experience only mild gastrointestinal upset, including heartburn, indigestion, and nausea. Patients who are elderly or who have a history of gastrointestinal bleeding are more prone to serious side effects, which include:

• Severe lightheadedness
• Severe or persistant headache
• Blood in stools
• Blood in urine
• Coughing or vomiting blood

The above warrant medical attention.

Pradaxa is known to interact with certain other medications. Of greatest concern are:

• Dronedarone
• Rifampin
• Ketoconazole
• NSAIDs, including over-the-counter varieties
